This study aims to examine the representation of Narcissistic Personality Disorder (NPD) in the film Joker. It employs a qualitative approach using Charles Sanders Peirce’s semiotic analysis model, which examines sign relations through the elements of representamen, object, and interpretant. The findings indicate that the character Arthur Fleck represents pathological narcissism through the construction of visual and narrative signs that emphasize fragile self-esteem, a need for recognition, social isolation, and aggressive responses to structural pressures. The film reflects a close interconnection between personality disorders, social trauma, and inequality within modern social systems.
